In the Collaboration Hub, we are promoting quality improvement as our single approach to enhancing the quality of Somerset’s health and care services. Our aim is to provide colleagues with the tools and support to develop their knowledge and skills in improving services for the people of Somerset. The focus is on service user experience, population health, care outcomes, colleague wellbeing, sustainability and optimisation. You can find out more by looking at our quadruple aim.
Having a shared approach to quality improvement means colleagues from across the Somerset health and social care system can work together on the whole of a person’s journey as they navigate different health and care services. This provides a more rounded view on what is working well, what could be better and presents new opportunities for collaboration.
Our quality improvement methodology is a straightforward, evidence-based, seven step process, proven to bring about sustainable change. It works every time and anyone can use it.
Training, coaching and expert support is available. Our team of highly skilled quality improvement practitioners, knowledge and evidence specialists, data scientists, transformation advisors, project managers and resident engagement lead, are here to support colleagues across the county. We love spreading quality improvement and change management wisdom and seeing what people achieve with it. Get in touch and we can discuss how we can support you.
